The Tracheostomy Toolkit is designed to empower individuals with knowledge and support to take control of their tracheostomy care. By standardising care across hospitals and communities, this toolkit offers 365-day patient support at home, and focuses on the high-value things that are going to make the biggest impact – leading to fewer hospital visits and more confidence for patients and caregivers.
