Who This Is For:

Supporting Patients & Your Hospital Teams

Our digital toolkits are designed to support the individuals using them as well as the healthcare professionals providing care, and created to easily integrate into hospital and healthcare environments.

  • Individuals with a tracheostomy and their families, helping them to confidently manage their condition.
  • Frontline community healthcare teams, equipping them with standardised tracheostomy care bundles for hospital, primary care and community teams.
  • Hospital staff, ensuring the highest standards for adult and paediatric tracheostomy care.
  • Commissioners and Clinical Leaders, providing them with data to demonstrate the impact and efficacy of the programme on patient care.

Why This is so Useful:

Easy to Procure & Easy to Implement

The Tracheostomy Toolkit is designed to empower individuals with knowledge and support to take control of their tracheostomy care. By standardising care across hospitals and communities, this toolkit offers 365-day patient support at home, and focuses on the high-value things that are going to make the biggest impact – leading to fewer hospital visits and more confidence for patients and caregivers.

The reporting mechanism for Commissioners and Clinical Leaders, demonstrates the impact and efficacy of the programme on patient care, delivering data about the uptake and engagement of the programme across all settings, as well as the measurable outcomes generated by the toolkit.
