All-Wales COVID-19 Secondary Care Guideline

The All-Wales Secondary care COVID-19 hospital guideline was created in early March as the COVID-19 pandemic gathered momentum. It was recognised that since this was a new disease, where evidence around the management of infected patients was scant, that we needed to create a guideline that was dynamic and responsive to emerging evidence.

We therefore created a guideline with two key components, a fixed aspect which reflected the general flow through the hospital of COVID-19 patients from admission, to COVID ward or ITU, or to discharge, and a variable component which encompassed management strategies at various stages of the pathway that were likely to change.

Triage of a patient with suspected COVID-19

In this video, Dr Paul Frost outlines the triage decision that is made when a patient presents to hospital with suspected COVID-19; those who need admission to hospital and those who can be discharged.... For the purpose of this classification system, patients should be classified according to the degree of oxygenation failure; severe and moderate failure who should be admitted, and no oxygenation failure who should be discharged and instructed to re-present to hospital if their symptoms deteriorate.

Change in Clinical Status: Escalating from Ward to ITU

In this video, Dr Paul Frost provides an overview of the decision to escalate a patient from a Ward to the intensive care unit. This decision should be made by a senior clinical decision maker when the SpO2 is... less than 90% despite maximal supplemental oxygen via ward based devices, and after discussion with the patient and their relatives.
